Working to help people leave hospital sooner

We are working with health and social care partners to help reduce delays for people leaving hospital after an inpatient stay.

Sometimes patients are ready to leave hospital but experience delays because different organisations need to work together to arrange care.

We are bringing teams together to improve communication and make the process smoother.

The aim is to help people return home or move to the most care placement as quickly and safely as possible.

Local pharmacies supporting stroke prevention

Community pharmacies across Greater Manchester are being given new resources to help prevent strokes and support people living with the effects of stroke.

Pharmacy staff often see people regularly and can help identify risk factors, provide advice about medicines and healthy lifestyles. They also encourage people to seek help if they notice stroke symptoms.

New information resources have been created to help pharmacy staff feel more confident in supporting stroke survivors and their carers.

New specialist gym in Bury

People recovering from stroke and neurological conditions in Bury are benefiting from a new specialist rehabilitation gym at the Blossom Suite in Radcliffe.

The facility has been made possible thanks to fundraising by local supporters, community donations and contributions from healthcare teams.

It provides a dedicated space where people can work on their recovery, build confidence and improve their independence.

The new gym is already being used for stroke follow-up clinics, specialist assessments and rehabilitation sessions, helping more people access the support they need closer to home.

Charity calls for better neurological care

Neuro plan now. Millions can't wait.

The MS Society is calling for a national plan to improve care and support for people living with neurological conditions in England.

The charity says a dedicated plan could help improve access to services and ensure people receive the care they need, regardless of where they live.

It is encouraging people to support its campaign and help raise awareness of the challenges faced by those living with neurological conditions.
