Partners in North Manchester come together to improve stroke discharges
Professionals from inpatient stroke and discharge services, local community stroke teams, adult social care and the voluntary sector recently came together at a Fairfield General Stroke Unit Discharge Pathways Workshop to explore opportunities to improve discharge experiences for people following a stroke.
The workshop focused on strengthening partnership working across Bury, Oldham, North Manchester and Rochdale, with discussions centred on how services can work more closely together to support safer, more coordinated and person-centred discharges. The workshop highlighted a shared commitment across organisations to improve outcomes for patients and carers, recognising that successful discharge relies on effective collaboration across the whole system.
The workshop identified a number of practical improvements that can be progressed in the short term, including clearer pathway information, named contacts across services, earlier discharge planning and stronger communication between organisations. People also highlighted the important role of carers and voluntary sector organisations, including the Stroke Association, in supporting people as they transition home.
Alongside these immediate actions, the event helped establish shared priorities for future improvement, including greater consistency across localities, stronger information sharing and continued collaboration between health, social care and community services.
