Stroke & neurorehabilitation community team referral

The network has worked with GM stroke and neuro clinicians to develop a new merged community referral form. The document was created by transferring the sections from the old stroke and neuro forms into one document and amending this through clinician feedback at the Rehabilitation Subgroup.

The document has been approved by the GMNISDN CEG and is now being used across all GM community stroke and neuro rehab teams. The form can be found below.
